The request to challenge the SRNA examination, your request must be approved, and exam taken within one year of completing the required nursing courses.

To be completed by Medication Aide Instructors requesting access to the KCTCS testing site to registrar students for the CMA I or CMA II Exams located at a KCTCS test site. (Not required for KCTCS Instructors)

Challenge application for Nursing students. The request to challenge the CMA I examination must be made, within one year of completing the required nursing courses. If the challenge is approved, the exam must be taken within 60 days. (Medication Aides from other states requesting reciprocity, need to contact the KY State Board of Nursing for requirements/instruction)

This form is used by facilities requesting facility role access to the TMU testing platform, It may also be used if there is a change in the person responsible for this role. (payments/reports) This is not the same as a MAP 414 change. This is only system access approval.
